EUREST SERVICES ('EUREST'/THE 'PROMOTER') CUSTOMER FEEDBACK SURVEY PRIZE DRAW (HEREINAFTER REFERRED TO AS THE 'PRIZE DRAW').

THIS IS A FREE PRIZE DRAW. NO PURCHASE NECESSARY; A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING.

Open to those customers of the Promoter resident in the United Kingdom, over the age of 18 years, excluding employees of, and other persons connected with, the Promoter and Market Force Information (Europe) Limited (see below under 'eligibility'). By entering, entrants agree that the Promoter and Administrator shall not bear any liability for, and entrants agree to release, indemnify and hold the Promoter harmless from, any losses, damages, claims, actions and any liability of any kind resulting from participation. Void where prohibited or restricted by law.

THE PRIZES

One prize of £100 will be awarded (in the form of Love 2 Shop Vouchers), once at the end of each calendar month period, for the duration of the Prize Draw promotion, subject to availability. One prize of £500 will be awarded (in the form of Love 2 Shop Vouchers), once at the end of each calendar quarter period, for the duration of the Prize Draw promotion, subject to availability
The prizes are not transferable and must be accepted as awarded. Any applicable taxes are the responsibility of the winner.

PRIZE DRAW RULES AND HOW TO ENTER

There will be a series of distinct and separate Prize Draws, lasting one (1) month and one (1) quarter. The initial Prize Draw period starts on 01/06/13 [n1] at 12.01am on the www.didwemakeyousmile.com website and such other websites as may be determined by the Promoter, from time to time, at the Promoter's sole discretion (together the 'Websites') and ends on 30/11/2013 at 11.59pm. Subsequent Prize Draw periods will follow, beginning on the 1st day of the month and ending at 11:59pm on the last calendar day of that month or quarter. Each subsequent Prize Draw will use the Websites. The Promoter, in its sole discretion, shall determine the number of Prize Draws to be held. The Administrator will post notice that the Prize Draws have ended on the Websites. Entrants will be included only in the relevant draw, only for the period during which their response was received.

To enter, you will be invited to complete a survey by a representative of the Promoter, by being given a survey response card at a participating location or by receiving an invitation on a till receipt at a participating location (hereinafter referred to together as the 'Invitation'). You will also be able to enter through other point of sale material visible in the participating location. You must complete the survey online on the relevant website. Each Invitation response will be submitted to the draw for the Prize Draw period within which it was received. To complete the survey on the relevant Website, enter the code found on your Invitation (including any letters and/or special characters).

This offer is limited to one survey entry per Invitation. Complete the survey within the designated selection fields, marked on the online Prize Draw form. All areas marked with an asterisk ('*') must be filled out before submitting your form. Where areas marked * are not completed, the survey will not be entered into the Prize Draw. For each properly completed online survey, the entrant shall receive one (1) entry to the draw.

All surveys and information become the copyright and property of the Promoter and will not be returned. Postcodes may be used by the Promoter for customer-mapping purposes. In the event of a dispute, the Promoter shall be entitled to randomly select a winner. The Promoter's decision is final.

For purposes of the Prize Draws, each person, household, group or organisation is restricted to one entry. This means that, no matter how many surveys are completed, only one of the responses will be put forward for a draw in any one Prize Draw period. Entrants leaving a common telephone contact number will be considered, for purposes of the Prize Draw, to belong to the same household, group or organisation and therefore be subject to the above restriction.

For the Prize Draw, the online survey must be received by the Administrator by the closing date of the current Prize Draw period.

ELIGIBILITY

To be eligible, you must be resident in the United Kingdom and over the age of 18 years. You are not eligible if, during the Prize Draw period, you are (or you live with) or your immediate family member (parent, child, sibling and spouse of any of the foregoing) is an employee, representative or agent of the Promoter or the Administrator or any of their subsidiaries, licensees or affiliates.

THE DRAWS

Every month, on the second business day following the last day of each calendar month period, the Administrator will hold random draws for each of the Prize Draws which took place in the preceding month. A prize of £100 in vouchers subject to availability with no cash alternative will be awarded to one randomly selected winner from each Prize Draw. In addition, every quarter, on the second business day following the last day of each calendar quarter period, the Administrator will hold random draws for the Prize Draw which took place in the preceding quarter. A prize of £500 in vouchers subject to availability with no cash alternative will be awarded to one randomly selected winner from each Prize Draw. The draws will be held at the office of the Administrator. If a selected entrant chooses not to accept the prize, the prize will be deemed to be forfeited and no substitution or other compensation will be awarded.

Within 14 days of the Prize Draw, the selected entrant(s) will be contacted by e-mail and must accept and return the conditions of the prize by completing (within 14 days of receipt) a declaration and release of liability. The declaration and release form confirms that the winning entrant complies with the Prize Draw rules outlined in this document and that he/she releases the Promoter and Administrator; its parents and affiliates; the employees, officers, directors, representatives and agents of each of the foregoing; the heirs, executors, administrators, personal representatives, successors and assigns of the foregoing, from any claims, demands, payments, actions or causes of action for damages, loss or injury, arising as a result of participating in the Prize Draw and accepting a Prize, which the winning entrant or his/her heirs, executors, administrators, personal representatives or assigns may now or hereafter have against him/her.

You can expect to receive the prize within 28 days of receipt of the declaration and release of liability,

If the selected entrant does not comply with these Prize Draw rules or has not completed the declaration and release of liability within the appropriate timeframe, that entrant will be disqualified and an alternative entrant will be randomly selected from the eligible entries received.

CHANCES OF WINNING

The chances of winning a prize depend on the number of eligible entries received and the number of the Promoter's outlets participating in the Prize Draw.

OTHER RULES

By entering, entrants agree to abide by these Prize Draw rules and all decisions of the Promoter will be final and binding on all entrants. This Prize Draw is subject to all applicable laws.

By entering, entrants agree that the Promoter shall not bear any liability for (a) entries which are lost, postage-due, stolen, illegible, incomplete (or otherwise improperly completed), delayed or destroyed (including, in the case of online entries, as a result of failure of the Websites or technical problems or traffic congestion on the Internet), (b) the loss, theft or misuse of the prize or (c) any property damage in any way attributable to this Prize Draw or the Prize (including, in the case of online entries, damage to any telephone network or lines, online systems, servers, access providers, computer equipment or software) or resulting from downloading any material from the Prize Draw Websites.

No correspondence will be entered into, except with the selected entrant who qualifies to win the prize. By entering, the winner grants the Promoter the right to use the winner's name, address, comments and/or likeness, without further compensation, in any advertising or publicity, except where prohibited by law. All entries become the property of the Promoter/Administrator which assumes no responsibility for lost, stolen, delayed, damaged or misdirected entries or for any failure of the Websites, during the promotional period, for any problems or technical malfunction of any telephone network or lines, computer online systems, servers, access providers, computer equipment, software, failure of any entry to be received by the Promoter/Administrator on account of technical problems or traffic congestion on the Internet or at any website, or any combination thereof, including any damage to an entrant's or any other person's computer, related to or resulting from playing or downloading any material in the promotion. The Promoter reserves the right, in its sole discretion, to cancel, modify or suspend the entry form download portion of this Prize Draw, should a virus, bug or other cause, beyond the reasonable control of the Promoter, corrupt the security or proper administration of the Prize Draw. In the event of cancellation, termination or suspension, at its discretion, the Promoter shall select a winner from eligible, non-suspect entries, received prior to the event requiring such cancellation, termination or suspension.
Any attempt to deliberately damage any website or to undermine the legitimate operation of this promotion is a violation of criminal and civil laws and, should such an attempt be made, the Promoter and Administrator reserve the right to seek remedies and damages to the fullest extent permitted by law, including criminal prosecution.

Surveys obtained through fraudulent means or through an abuse of the Prize Draw or in any way tampered with, forged, mutilated, illegible, incomplete or mechanically reproduced may, at the sole discretion of the Promoter and/or Administrator, be disqualified.

The Promoter and/or the Administrator may collect personal information for the purpose of administering this promotion. The Promoter and the Administrator will not share any personal data or details with other organisations. In administering this promotion, the Promoter and/or the Administrator will not collect more personal information than absolutely necessary and will not retain it after the duration of the promotion unless otherwise agreed by the entrant. By completing the survey and entering the draw the entrant agreed that the Promoter and/or the Administrator may process its personal data for the purposes of collating survey results and operating the draw.

Except as provided herein, the personal information which entrants provide to enter this Prize Draw will be used only to administer this Prize Draw, unless any entrant indicates that he/she would be willing to participate in future surveys. Unless otherwise indicated, the responses to the survey questions which any entrant provides will be used by the Administrator in an aggregated manner only and will not be linked to any entrant's personal information. The Promoter reserves the right to terminate the Prize Draw at any time, at its sole discretion, and makes no representation as to how many Prize Draws will be held.

For the name of the winner for a specific monthly Prize Draw, send a self-addressed envelope to Eurest Food, Customer Satisfaction Survey Prize Draw, C/O Market Force Information (Europe) Limited , Mount Mill Farm, Wicken, Milton Keynes, MK19 6DG, within thirty (30) days following the last day on the month in which the Prize Draw closing date falls. You must specify the applicable period for which you are requesting a winner's name. The winner's list will be sent only after the prize has been awarded
